Cadence support for child workflow executions (#187)

* IDL changes to support child workflows

* Schema and persistence layer changes to support child executions

Added parent_workflow_id, parent_run_id and initiated_id to mutable
state for workflow execution.  These can be used by history engine to
communicate close result back to parent execution.  Also added
child_executions_map to executions table to keep track of all pending
child executions.
Persistence layer API now supports passing in parent execution info on
creation of new workflow execution.  GetWorkflowExecution now also
returns outstanding child executions as part of mutable state.
UpdateWorkflowExecution API now also support upsert and delete
operations for pending child executions.

* History engine changes to support ChildWorkflow executions

Added new API on history service for creating decision tasks for
workflow executions and recording child workflow execution completion.
Updated StartWorkflowExecutionRequest to also have ParentExecutionInfo
when it is a Child execution.
Schema changes to also store domain_id for parent workflow execution and
completion_event for current workflow execution as part of mutable
state.
Added new type of transfer task for processing of creation of child
workflow executions 'TransferTaskTypeStartChildExecution'.  Transfer
queue processor implementation of StartChildExecution transfer task.

* Also pass parent execution mutable state when creating a new execution through ContinueAsNew
